Connecting with Acute Care:  New EMR IDs Available for Home Care Clinical Visit Documentation

October 9, 2014 - Attention Home Care Directors and Integration Specialists:

Providing home care data to the acute environment is vital to maintaining a complete picture of your patients' health, especially if they go to the ED or get admitted to the hospital. 

Previously, acute care providers were only able to determine whether home care visit information was documented as home health or hospice. Now, discipline-specific EMR IDs are available for use within Home Care Clinical Visit Documentation.

Hospital personnel will now be able to see:

• Visit discipline, such as Nursing, Physical Therapy, Occupational Therapy, etc.
• Service Line, such as Home Health or Hospice.
