Lodi Memorial Hospital Succeeds with MEDITECH’s Multidisciplinary Discharge Routine

Wendy BurnsJuly 18, 2014 -  By utilizing the centralized 5.66 Multidisciplinary Discharge routine, care team members at Lodi Memorial Hospital (Lodi, CA) seamlessly communicate to improve patient outcomes and fulfill Meaningful Use requirements.

In MEDITECH’s MAGIC & Client/Server 5.66 releases, the Discharge process was redesigned into a highly interactive processing tool—providing Lodi Memorial with the opportunity to implement a standard workflow hospital-wide.

“With MEDITECH’s enhancements, every discharged patient now has a comprehensive set of instructions with an up-to-date medication list in hand,” says Wendy Burns, director of information technology and health information management at Lodi. “Giving our patients the information they need to manage their conditions outside of the hospital is one of the first steps to keeping them healthier, longer and preventing readmissions.”

In addition to streamlining a patient’s discharge, this new routine helped Lodi Memorial with processes they historically struggled with, including medication reconciliation and the Problem List. Ease of use, complemented by efficiency, makes the Multidisciplinary Discharge routine a standout feature with Lodi Memorial’s physician usage around 85 percent.

On-the-Road Seminar
On March 4, 2014, Lodi Memorial hosted a 5.66 Discharge Seminar with MEDITECH’s Kerri Nash, project coordinator, and Applications Specialist Jamie Locke, RN. Feedback revealed this session was very valuable, and other hospitals appreciated the opportunity to learn from one another. Burns credited the small and intimate setting for allowing attendees to get an in-depth understanding of the Multidisciplinary Discharge routine, learning from both MEDITECH experts and peers.

“I really liked the discharge manual that was provided—it was very well done and easy to follow,” says Lodi Memorial IT Clinical Analyst Lisa Donati, RN. “It is always great to be able to network with fellow MAGIC folks and to share ideas. Overall, it was a great day.”

MEDITECH plans to facilitate similar seminars in the spring of 2015 for new 5.67 functionality.

Moving Forward
With the success Lodi Memorial experienced with Multidisciplinary Discharge, they are looking forward to future collaboration with MEDITECH. “It’s a good system and it works,” says Burns, “We appreciate MEDITECH and what they’ve done over the years—I’ve seen the company change and grow.”
