Health on the Go: Mobile Rounding and the Patient and Consumer Health Portal

Mobile Rounding

Q: How does a physician sign into mobile rounding when they have access to multiple facilities?
A: It is a 1-to-1 ratio, so a site would need one url, file server, and webserver per MIS. For a multi-facility site, they would want to review how many MIS databases they have set up

Q: Users want the ability to edit documents via mobile rounding. What is the status of this enhancement?
A: There is not a current proposal submitted that will allow for providers to edit documents before signing them within mobile. There is, however, a proposal to allow for documenting notes within mobile.

Q: What are the hardware requirements?
A:
Mobile rounding requires a web server, file server, and SSL certificate.

Q: Will we be able to manage our logon credentials?
A: Yes, it uses NT logon.

Q: How extensive is the build?
A: In terms of dictionary build, there is no additional building required because the information is pulling directly from EMR. During the implementation, you will have to provide MEDITECH with users that are set up as Super Users. These Super Users will in turn set up all other users of MOBILE, including physicians.

Q: Do ITS reports flow to mobile rounding?
A: If they appear in Clinical Review, they should be visible. The same is true if you are on the Client Server platform utilizing EMR. The reports tab will display reports from PATH, ITS, and PD for CS; PATH, RAD, DPT, ITS, and PD for Magic as they would display on the Imaging, Pathology, and Other Reports panels in EMR and Clinical Review. Any embedded images will be viewable on reports in Mobile, including PACS images.* Users have the ability to select the clipboard on the right to view the report.

Q: Are new results highlighted in red as they are in EMR?
A: No, all results will appear in the same color, however critical results would be flagged and shaded.

Q: Is there a paging functionality for critical or new results?
A: No, there is not a current functionality in mobile that will provide a paging functionality.

Q: Will coding done via mobile rounding be communicated to a third party billing system?
A: There is no automation to a third party vendor for coding in mobile.

Q: Will the sign function on the iPad communicate back to ICRs?
A: Whatever appears in the sign queue in EMR will display.

Q: Will all of the functionality demonstrated in this session be available in early 5.66?
A: Yes, all enhancements shown will be included in the initial release and priority pack for version 5.66. The enhancements are currently coded for Magic and development is currently working on completing the enhancements for Client Server.

Q: Will documenting patient reports be available in the future?
A: Yes, along with routine ordering.  A focus group has been created to develop the ordering capabilities, and there are future plans to allow for documenting in the application as well.

Q: How do you see images in the Mobile Rounding product?
A: Mobile rounding works the same way as the applications do, in regards to viewing images. You would either see the images from the PACS vendor or they would have been scanned into the system. Everything is directly pulled from EMR or Clinical review.

Q: Will MEDITECH be adding Physician Documentation to the Mobile Rounding product after Medication Ordering is added?
A: There are currently discussions on-going at MEDITECH about the addition of adding Physician Documentation to Mobile Rounding.

Q: Is Mobile Rounding a separate module from the Physician Desktop module?
A: Yes, they are separate modules that are both part of the PCM suite of applications.

Q: Are there function keys in Mobile Rounding as there are in MAGIC navigation?
A: Mobile Rounding utilizes browser based functionality, so function keys are not necessary in the product.

Q: When electronically signing on a mobile device, can you control if the physician can sign using a password or pin number?
A: Sites have the ability to determine whether providers use a PIN or Password in the Universe parameters. This is not specifically geared towards Mobile Rounding, rather the setting will affect all areas of the MEDITECH system for which eSign is available. Regardless of whether Password or PIN is required, the eSign button in Mobile Rounding will say “PIN,” which is consistent with what will display on the sign list in EMR or Clinical Review as well.

Q: E-signing on a mobile device, is there the ability for the physician to reject a report incorrectly assigned to them?
A: Physicians cannot reject a report incorrectly assigned to them from Mobile Rounding. The reject option is only present in the Orders sign list, with the reason for rejection comment list as defined in the OE parameters. This is the same list that displays when rejecting orders for E-sign in other Meditech applications. There is a proposal to request this functionality, which has been updated to note additional interest in the request.

Q: Does mobile rounding communicate with IMO in any of the diagnoses fields?
A: Mobile Rounding Coding mimics Coding functionality in PCM. The diagnosis field in PCM does not communicate with IMO, thus it will not in Mobile Rounding.

Evaluation and Management, often referred to as E&M coding, defines the amount of work a physician performs when they have an encounter with a patient. It’s a tool for providers to use to get reimbursed for their time and work with a patient and relies on build in the PCM Classification Dictionary and Level of Care Dictionary for use in Physician Documentation. For Mobile Rounding use, the Encounters Dictionary must also be built. More information on setup can be found here.

Q: What applications are needed in order to use Mobile Rounding?
A: Mobile Rounding is not an individually purchasable product, rather it comes as part of your PCM suit of applications. Thus, a licensed copy of PCM is required and sites interested in using Mobile Rounding must be LIVE with EMR or Clinical Review. Users of Mobile Rounding must be providers with access to PWM and EMR or Clinical Review.

Q: Can you launch another session to a URL from Mobile Rounding to connect to the PACS system?
A: No, however you could have another session open to the PACS software. We are working with development on providing access to PACS images, but currently you can only link to scanned images.

Q: Could you access the reports in Medical Records for viewing the PACS images?
A: You would have to access the Medical records report in another MEDITECH application such as ITS, then view the PACS image, or open another session on the tablet to launch the PACS software and view the image.

Q: What integration is there with PWM on the task list?
A: There is not a task list feature within Mobile Rounding, at this time. The physician would need to access the task list through PWM as they do now.

Patient and Consumer Health Portal

Q: Can patients discontinue home meds they are on?
A: Currently, the Portal will only display the list of the home medications. They are unable to discontinue these medications through the Portal.

Q: Can patients update their past medical history?
A: At this time, patients are unable to update their past medical history.

Q: Is there notification to the patient when there is new activity for him/her?
A: An email will be sent to the patient daily indicating that there has been new activity for the patient. The patient will then need to sign onto the portal to view this information. Upon signing in, the patient will see a “What’s New” table that lists the areas with new activity, as well as red dots indicating what sections have new activity.

Q: Will the Patient Portal include information entered in LSS?
A: There is a combined Portal that includes LSS information. If your site is using LSS and has the combined Portal, then patients will be able to see information found in LSS such as allergies, home medications, conditions, appointments, and past visits. There is a parameter setting to enable MPM in order to use the combined portal.

Q: How is MEDITECH confirming that the formatting of the CCD works with other vendors it is shared with? How are we sure it works from the Patient Portal?
A: All MEDITECH platforms are certified for Meaningful Use Stage 2. Certification for MU2 requires that all EHR vendors use the same CCD specification for exchange. MEDITECH is also certified to use Direct Messaging, which allows the secure sending of information to a provider outside of your Health Information Exchange or lacking an EHR. With both of these certifications, MEDITECH can ensure we are ready to send the CCD in a safe and secure method.

Q: Is the Patient Portal only available in English?
A: Currently, the portal is only available in English. Users have the ability to change the settings on the browser to Spanish, which would then allow the patient to view most of the portal in Spanish.

Q: Can the Portal’s text be made bigger for visually impaired patients?
A: Not through the portal directly, however you could change your settings on the computer or in the browser to enlarge the display. Also, a proxy user, such as a family member, could be established to assist with the patient gaining access to their data online.

Q: Can the portal be setup so that it connect’s data from multiple EHR vendors?
A: No, the data is specific to an HCIS on the MEDITECH system.

Q: Is there a way to put an explanation next to an abnormal lab result in the Patient Portal?
A: Not at this time. The recommendation would be to use the filter feature to determine which labs come over to the portal. Also, customers could customize the text box on the screen to say “If you have abnormal results or questions, you can contact the following number, etc.” This message would display for all patients.

Q: How is MEDITECH ensuring security of the PHI data and printing of the data through the Patient Portal?
A: MEDITECH is ensuring the security of PHI by allowing the patient to see an audit log on the portal that will show the patient if they, or their proxy user, have downloaded the CCD.  The facility can also run the MIS Audit Logs to see if the Health Summary has been transmitted, along with determining which sections of the Portal have been viewed. MEDITECH has been certified by Rapid 7, a security and risk management firm, for vulnerability management and penetration testing.

Q: Where do the Patient Portal lab results pull from?
A: The results area of health record provides the patient with Lab and Microbiology results. Results that display in the Patient and Consumer Health Portal are pulling from either EMR for our Client Server facilities or Clinical Review for those on the Magic platform. There are a few conditions and control to determine what will display in the Portal. Confidential orders and results do not display in the Patient and Consumer Health Portal. There is also a Portal parameter that allows your hospital to determine the number of hours after a test is resulted that it displays on the Portal. The lab delay hours is a global setting and is not defined per physician or test.

Q: How is the CCD transmitted from the Patient Portal?
A: When the patient selects the Send Health Summary button, they are brought to a page that explains that the patient is responsible for obtaining an ehealth exchange address for the provider they wish to send the CCD to. The Patient Portal uses HIE Direct Messages to transmit the CCD.

Q: When can patients see LAB results...x hours after results available, or does a physician have to review the results and "confirm" that they're ok for patients to view?
A: There is a parameter to define the timeframe after which results are available for a patient once they've been completed in LAB.

Q: If a parent is signed onto the portal and looks at his/her results, then looks at his/her children's results, will that count as one time or more than one time for the Quality Measure.
A: The parent will count as one user logging into the portal. As a proxy user, they are counted as an 'authorized representative' within the portal.

Q: How do physicians see direct messages from patients?
A:
They will appear on the physician's desktop for the physician to read and respond to the patient.
